Complete Dogs agrees to exercise due diligence & reasonable care, & to keep the premises sanitary & properly enclosed. All dogs are handled or cared for without liability on person’s, other dog(s) or property by said dog, or from other unavoidable causes, due diligence & care having been exercised. Should any dog(s) become ill or seem to be in need of medical attention, Complete Dogs reserves the right to administer aid and/or to use any available veterinarian if unable to contact owner or if deemed by Complete Dogs, delaying medical attention to contact the owner would risk proper recover. All expenses so incurred shall be paid by the owner of said dog in addition to any other applicable fees. Owner agrees to indemnify & hold harmless Complete Dogs from any liability arising from such veterinary charges. A yearly check up for general health and a stool test for parasites such as Giardia is required. Owner agrees to pay the rate for service in effect, on the date & time the dog is checked into Complete Dogs. Prices are subject to change without notice. Late cancellation fees will be applied for reservations cancelled 72 hours or less prior to scheduled reservation. Should my dog not be picked up at the specified time I authorize Complete Dogs to take whatever action Complete Dogs deems necessary for the continuing care of my dog & I will pay all costs & charges thereof. If I fail to pick up any dog(s) within 5 days following the scheduled pickup date & fail to contact Complete Dogs within those 5 days & pay Complete Dogs for the extended stay, I agree that Complete Dogs will release the dog(s) after that period to a place as Complete Dogs sees fit, such as Humane Society, Rescue Agency or Foster parents. Owner shall remain liable for all daycare bills & other applicable charges occurred in the care & maintenance of the dog listed in this contract. The Owner further agrees to be solely responsible for any & all acts of behaviour of said dog while in the care of Complete Dogs. The Owner of the pet agrees to pay any & all legal fees incurred by Complete Dogs in the collection of any daycare services, training fees, or other charges accrued by the Owner of the dog. Pets arriving with fleas/ticks will not be accepted. Sick and/or injured dogs will not be accepted into daycare unless previous arrangements have been made. For sick or injured dogs that have been left by owner without informing Complete Dogs about illness/injury before drop-off an additional minimum charge of $25.00 per day applies. Complete Dogs reserves the right to refuse any dog. Pinch collars, choke chains, flea collars & retractable leashes are not permitted on the premises. Owner agrees that the dog will be picked up by the owner or other pre-approved person during our reception times listed on our website www.completedogs.ca case the owner is unable to pick up the dog(s) by closing time in the evening the dog(s) will be kept for an extra overnight stay. There is an absolutely no pick-up policy of any dog(s) after hours. There is a strict NO REFUND POLICY once payment has been received by the daycare. The dog’s nails must be trimmed short. An additional charge of $10.00 for the initial clipping & $ 5.00 per weekly maintenance clipping applies for a nail clip at Complete Dogs. Long nails are not only uncomfortable & dangerous for your own pet; they are also a hazard when playing with other dogs & our staff. My dog may be videotaped, photographed & recorded when on Complete Dog’s premises and Complete Dogs shall be the exclusive owner to the results & proceeds of such taping, photography & recordings with the rights, throughout the world, of unlimited use, & to copyright, to use license in any manner. I will abide by the rules & regulations of Complete Dogs provided to me and/or posted from time to time at Complete Dogs premises. ü The 'Customer Information Form' above & vaccination records must be received before scheduling any service. ü Dogs are required to come for a free visit to evaluate temperament. Typical reasons dogs fail are excessive: Aggressive Behavior ~ Separation Anxiety ~ Excessive Mounting ~ Biting of other dogs or staff Excessive barking ~Severe medical problems ü Dogs must have vaccinations including rabies, DHLPP (distemper) and be current with their Bordetella (kennel cough) vaccination. ü Dog must be on a flea and tick preventative from May-November. ü All dogs should be leashed or under control when entering or exiting Complete Dogs. ü All dogs that are not spayed or neutered may be subject to separate conditions. ü Passing Certificate from Complete Dogs Basic Obedience Training or equivalent preferred for Daycare and Boarding. ü IF YOUR DOG IS EXHIBITING ANY of the following you may be refused service. ADVISE BEFORE VISITING: Coughing, Vomiting, Diarrhea, Lethargy, Green or Yellow Nasal Discharge Declaration: At Complete Dogs our goal is to provide a safe, fun & happy environment for your Dog. However, dogs are animals, & as such, can be unpredictable & have the potential of displaying unwanted behaviors without warning. Working with dogs is not without risk to students, family members, guests, other people, dogs & possessions. Assumption of Risk: By checking below, the client hereby agrees to indemnity, agrees to hold harmless, & to release from liability; Complete Dogs, and/or its principals along with all other related names & combination of names, employees & agents for any & all risks involved with any & all products and/or services of the facility, any & all direct or indirect results thereafter. Indemnity: By signing above, the client agrees to indemnify, to hold harmless, & to release from liability; for any claim by any person or company against Complete Dogs, and/or its principals along with all other related names & combination of names, employees & agents including the legal costs to defend any such claim, whether or not the damages are caused in whole or in part by the negligence, actions or inaction’s of any of these parties. If we are unable to contact you or your emergency contact and decisions need to be made for the care of your dog, you agree to let Complete Dogs make those decisions on your behalf and are responsible for any incurred expenses that occur. Any images of your dog taken while in out care, belong solely to Complete Dogs. Release from Liability: By your signature affixed, you agree and understand all of the terms contained within this Contract, Agreement & Release.
